New Patient Consultation
Service Description
This 90 minute appointment includes a deep dive into your past medical history, your current concerns and symptoms, a physical assessment, recommendations for labs or other means for trying to identify a root cause for your symptoms. There may also be a treatment plan initiated which could include lifestyle recommendations, supplements or medications. There is no commitment with the initial consultation to have a follow up appointment. You may find that one visit is all you need to gain clarification on your health and receive direction for treatment. Often times lab work or other testing is recommended in the new patient consultation to help gather information in identifying underlying issues. A new patient follow up visit is generally scheduled to review lab results in detail and continue developing a treatment plan. After completing a new patient consultation and new patient follow up, you will then be considered an established patient and can use the established patient pricing for appointments that can be scheduled as needed. Alyssa is a primary care provider and can manage long term health needs if you would like to continue seeing her as needed for health concerns that arise. *We do not bill insurance but you may use an HSA/FSA card if you have one or request a super bill for insurance reimbursement (not for Medicare or Medicaid). Cancellation Policy
We ask that you please allow at least 48 business hours prior to your appointment to cancel or reschedule. If it is within 48 business hours of your appointment, there will be a $50 cancelation fee. For new patients and discovery visits there is a $50 non-refundable deposit due at the time of scheduling to secure the appointment. We appreciate your understanding.
