This is the appointment for existing patients to schedule if you need to continue a plan of care or discuss a new concern.
You may have labs that need to be reviewed or a new lab order to obtain.
We may be following up on treatment recommendations from your previous visit and would like to discuss any improvements in your health or areas of concern that still need to be addressed.
*We do not bill insurance, but you may use an HSA/FSA card or request a superbill for insurance reimbursement. (Not available for Medicare or Medicaid) See Office FAQ.
We ask that you please allow at least 48 business hours prior to your appointment to cancel or reschedule. If it is within 48 business hours of your appointment, there will be a $50 cancellation fee. For new patients and discovery visits, a $50 non-refundable deposit is due at the time of scheduling to secure the appointment. We appreciate your understanding.
